Regarding Insurance Billing and Funding for Services:

I can bill/work with

• Cigna

• Anthem

• Aetna

• United Health Care

• UMR.

 

However, all plans are different and may have varying deductible or copay amounts. Submission of a claim is not a guarantee of payment by the insurance carrier. If a client's insurance denies any claim, I must subsequently bill my client for services provided. I am also considered in or out of network with the insurance carriers above based on the parameters of each individual plan.

Aside from insurance, I am also on several EAP (Employee Assistance Program) panels, including:

• Ski Corp (3 sessions for all mountain employees)

• Compsych (EAP)

• REPS (Reaching Everyone Preventing Suicide)

I do not work with the following:

• Medicaid

• Medicare

• CHP+

• Rocky Mountain Health Plans

• Keiser Permanente

• Angelhealth

My standard fee is $175/therapy hour (50 minutes). I provide a scaled fee as well, based on a person's ability to pay, with the following increments: $175 / $150 / $135 / $115 / $100.
