By law, I must have clients fill out the documents below to officially enter treatment services. There is a button below that has a link to print these documents prior to the first appointment. This saves time in the intake process. These documents can also be emailed or filled out at the first appointment. I ask that clients please come 15 minutes early to their first appointment if they wish to fill out these documents then.
